Seasons Evaluation of Group Assignments

Please submit this form for each member of your group and yourself.   After completing questions 1 through 9 for a group member, click on the submit form button and return to the form to complete for your next group member.  Don't forget to evaluate yourself!  The rating scale is at the bottom of the form. This information will not be shared with your group members.  I will average all responses and then make the final determination as to the grade for each assignment.  Be fair!

SCALE: 
5 = Provided an excellent contribution to the group assignment demonstrating a clear understanding of the project requirements.   Submissions exceeded the minimum number to complete the assignment and were timely.
4 = Provided a good contribution to the group assignment demonstrating an understanding of the project requirements.   Submissions met the minimum number to complete the assignment and were timely.
3 = Provided some contribution to the group assignment.  Submissions met the minimum number to complete the assignment and were timely.
2 = Provided cursory statements with not alot of substance.  Submissions met the minimum number to complete the assignment but were tardy.
1 = Little or no contribution to the assignment.  Submissions did not meet the minimum and were late.
